At the University of Texas at Austin we have our students complete an
Authorization and Release form electronically. The student would need to submit
their name, student ID number, a list of authorized representatives and their
contact information. Once we have that information we add it to that students
case file. Here is the link that students can use to complete that electronic

Hello Everyone,
I hope that everyone’s academic year has wrapped up by now and summer brings on
a bit more a dull roar.
I was curious if anyone would be willing to share their FERPA release they use
with students. Specifically, anything you have students sign so you can talk
with parents or release records to another institution would be fantastic. We
are developing something at KU and it would be great to see what our peers are
doing.
I appreciate any help you can offer!
