I am also quite sure that there were no EVAs between Earth and Lunar orbit. As in the movie, I believe that they did use the LEM for a time on the Apollo 13 mission. Houston would have probably nixed any EVA to evaluate the problem, as doing so would have used up precious oxygen during cabin re-pressurization, and further drained the batteries. My, how technology has improved! Now we just need to build a lunar colony...
