Our Mental Health Service is moving towards a centralized intake process
(i.e., all patients will first go through a centralized mental health intake
clinic as opposed to go through our separate mental health clinics). We are
considering having our patients complete a packet of self-report intake
questions/measures as part of a first step of the intake process. If your
station uses a comprehensive packet, could you forward it to me for us to
review?
