There will be a poster session at the upcoming Medical Home Summit - if
you are attending, this is a great opportunity to share your projects.
Please consider submitting a poster abstract for this widely attended
conference. Thanks, Kate

Veteran Centered Medical Home Summit Poster Session
April 13-15, 2010
Las Vegas, NV
**Short Notice!**
Posters may cover any topic related to projects or initiatives involving
the Patient Centered Medical Home or New Models of Care. We are
thrilled to have you participate in this unique opportunity to network
and share your ideas, successes and challenges.
The first 175 poster abstracts submitted will be accepted. Posters will
be available for viewing Tuesday, April 14th and Wednesday, April 15th.
There will be a dedicated poster session with refreshments on Wednesday,
April 15, 2010 from 4:30pm to 6pm. All posters must be removed by 7pm
on April 15th.
Posters should be no larger than 3.5 x 7.5 ft. Poster presenters are
responsible for transporting their poster and posting materials (tacks
recommended) to the conference site.
*Poster presenters must be registered for the conference in order to
display a poster.*
