Our preferred method of managing review feedback here is to provide a review 
spreadsheet with each document.

The reviewer fills in their feedback, including section number, page, etc, and 
their name. This way all reviewers can see what previous feedback has been 
given, and clashes are easily identified and the affected reviewers can be 
brought together to discuss and gain consensus.

If there is a lot of feedback, I like to send the sheet back with my comments - 
that way if I'm not going to make the changes they want they are forewarned, 
and we can work it out while I'm making the rest of the changes. Reduces all 
the to-ing and fro-ing when the next version goes out for review, because the 
reviewers know what to expect.

I've always found Word change tracking the nightmare that you seem to have 
encountered.   I'm sure there are some people that can use it properly on this 
list, but I doubt there are many if any who can guarantee your reviewers are 
going to stick to workable rules on it.

Do you have access to Acrobat Pro ?    The later versions of it (7+ if I 
remember) allow you to send out the PDF with commenting enabled,  and then it's 
possible to generate a summary of the comments from different people, 
identifying who suggested what change.
Haven't seen latest version of Acrobat Pro, but I understand it has enhanced 
methods for tracking change suggestions and keeping track of who's read a 
document, who hasn't, etc....

Otherwise, there are some cheap and/or free PDF readers that permit comments 
which you might be able to use.
I like the PDFXChange viewer for all kinds of handy things.  The free one I 
think is for personal use only and has some limits, but
the commercial version is relatively cheap and might be worth it.

You still have the issue of whether you send out one document on a circuit,  or 
a copy to each reviewer.   But that's always an issue, and your answer to that 
problem rests with your local circumstances (and timing etc).
The advantage of the PDF use is that you simply keep your original word version 
and fully control what changes are made.

I hate the idea of handing Word files to other people who generally stuff up 
the styles etc anyway....
