Here is a direct quote of the very first sentence in the lead story on the 
front page of The Sydney Morning Herald of Monday 26 March 2012:

“A police critical incident investigation was under way last night after a man 
was killed by an officer for the second time in a week.”

Whatever do they teach at journalism school these days.
