According to the ICC specificaion, all tables recorded in the profile are relative, i.e. they map "media white" to CIELAB [100,0,0] in the PCS. Absolute intent needs to be computed by the CMM, which applies the profile, from the relative tables. And for reflective prints, "media white" is typically the color of unprinted paper. I could image special cases though, where "media white" different from native paper white may make sense. For instance, if one creates a proofing profile, which emulates e.g. SWOP on an inkjet printer, then "media white" of such a profile would rather refer to the emulated paper color, and not the native inkjet one.
