Kip, Have seen this as well. Try moving your exit locations closer to the center of the column. Click on the lettering, then click the X in the center of the letter and it will show you the exit location. The trimmer selector is not picking up the top thread and as a result the knife is cutting it short causing it to springboard up and out of the needle.
