Industry standard is usually $1 per thousand for embroidery and keystoning 
garments (double price).  Its going to depend on what the market will bear 
in your area.  Some places charge twice as much while others charge less.  I 
charge $.50 per thousand on contract work and on my retail customers I don't 
really give a price per thousand but rather just figure what I want to make. 
As an example if I sell 100 polos that cost me $10 each I would sell them 
with left chest embroidery for around $20 - $23 each depending on the logo. 
I add $3 for names.  Most caps I sell for $8 - $10 and again depends on qty 
(most my customers order between 24-72 caps).

You will probably get quite a few different answers on this open of a 
question so you will need to figure out what works for you.
