I always put it on the left. 1 1/2 inches toward the button hole when cuff
is folded in half - 1/4 of an inch above the seam
1/4 of an inch in height.

LuAnn,
Sometimes people will put it on the right sleeve cause most people shake
hands with thier right so you can see the logo when shaking hands. I have a
customer that prefers his on the right even though left is more of a
standard :)
Its not a bad idea to ask the customer and if they don't know then do the
standard left.
