Do I require her full Proper name on the Oaths under the authority of the
people?  Is the fiction name making the oath? Or is it impossible for a
fiction name to declare an oath?

If you look at the U S styles manual, the use of all caps or middle
initials is not allowed.

What is the proper form for the name on an oath of office?

I have some in hand that are Proper, some are Proper with Middle Cap
initial, and some ALL CAPS.
