Also, on the ds11 they ask if you were born in a country other than the
United states where was it? I believe you should write in the State you were
born in. Each state is actually a country.
Also, above the ss number write "for information only."

in America? But on the DS-11 you would have to check YES to citizenship
and provide the number.
The answer should be the same. NO...as LONG AS that "person" does NOT
work for the government the "rule" of being able to work for a living
applies...no "number" is needed.
The same is true IF you were born on a military post or on any place that
is owned by the US government...including embassy's and ports.
