accessible-iOS Re: VO volume on iPhone 5s after updating from 4s

Can someone explain why VO volume gets stuck?  What exactly should I tell 
Apple?  Why does this work fine on my 4s and not on the 5s?  

Thought that by waiting a month that this behavior would be fixed.  :(
