The app by Dynamic App Design called Metronome Plus works swell with Voiceover and you can pay $1.99 to upgrade to get the in-app purchases but it is basically free.

Ok here is an update, I found the one called Metronome Plus by Dynamic App Design and it is free from the app store, just contacted the developer and asked if they integrated voiceover accessibility and they said they have, so I'll test it out and get back to you all to see whether or not this is true, and if not true, I'll encourage them to make it accessible to VO users.
