I'm guessing this is in Office Word? U is certainly the shortcut for underline, and ALT-o is an Office accesskey. So something might be pressing ALT by mistake? The other thing it might be is StickyKeys - http://en.wikipedia.org/wiki/StickyKeys Get her to try pressing both SHIFT keys at the same time, then see if it still does it.
