I notice that if I adjust the volume when reading a label the Penfriend
doesn't go to the maximum volume when reading another label.  Instead it
keeps the current position.

Trace,

The plus and minus button is for the volume.  Each time you read a new 
label, (at least in the earlier versions, of which I have one) the volume 
raises to its maximum, which isn't too good;  but, if it's a long item 
you're reading, you can at least turn it down with this button.  Keep 
pressing and you should get what you want.
