Thank you very much for your suggested way forward.  I followed the link on my 
receipt as you suggested and submitted a report.  There was then a message on 
screen to say that someone would be in contact in three business days.

Hi Andrew

When you receive your receipt from iTunes via email, there is a link to click 
on what you can report a problem. You can only report a problem once you have 
been charged for the item, at least that's my understanding however you could 
try going to reportaproblem.apple.com and log in with your Apple ID. Good luck!
