No idea whatsoever what happened.  One minute the spirit level was going
fine, then we hit silence.  I did the usual things to check, but nothing
seemed to work.  I put the iPhone on my Mac, and VoiceOver was checked in
the accessibility option.  Eventually, I tried headphones, and got speech,
but nothing if I unplugged them.  I did a continuous read, and played with
the volume until I struck gold! I have never seen that happen before, but at
least I have discovered a couple of new things to try if it ever occurs
again.

Hi. Did you accidently tripple tap with 2 fingers? This mutes speech but
voiceover is still running and you have to perform the same gesture to get
speech back.
