All this discussion prompted me to actually sort my form out. I'm in the West Yorkshire lucky region. The way it works here is that we have a two week window corresponding to our surname in which to fill out the form and hand it in with a photograph. I've just been to the central post office where a member of staff filled out the form for me. I showed my current pass and signed the completed form and that was it. I wait until March for the pass to come through. So it appears some councils are either on the ball or jumping the gun while others leave things to the last possible moment which potentially leaves their Disabled residents at a disadvantage.
