Hi,I helped design the See It Right guidelines and RNIB's internal guidelines about things like this some years ago. In my experience there are two approaches to forms that can work. One is to simply do a text based form with all the questions down the left and no dots or dashes or anything so one can answer to the right. The other is a two column table where you can right answers in the right hand column. Both should be OK these days. Obviously, for renewal, an HTML form would do.
