I say the major difference for most people between Home Basic and Home
Premium is that Windows Media Player is only in the Premium version.  Also
included in Premium is the ability to make DVDs. Not a major problem if you
have something like Nero.  Many of the other differences tend to be visually
related, such as the Aero desktop. Nice to look at, but may only impress
your sighted friends.
