I don't have an iPhone, Saqib, but it sounds as if Siri is hearing what the
iPhone is saying.  Could you try plugging in headphones so any feedback from
the phone won't be picked up by Siri?

Hi. When I attempt to reply to a message by using Siri! Voice over
will speak giving instructions and they end up being apart of the text
message that I'm replying to. This usually happens 2 times out of 5.
