It's easy to blame the adaptive side of things, but speaking from a
technical support side, in 9 cases out of 10, it is sadly some 3rd
party application which is the cause.  And remote debugging cannot
always solve the problem either.  And to be honest, I quite enjoy
finding out what these problems are.

In a recent case, with some software which cost just under a grand, it
appeared to freeze on me.  After a protracted exchange with the
developer in California, it turned out that while I had the
dictionary/spell checker in their product set to American English, my
own Windows 7 was set for Brtish English.  Set both to the same, and
the problem went away.

And this was a major international company who distrubute world-wide.

And why are so many people (who DO NOT use screen readers at all)
complaining about loss of sound after an update to their XYZ player?

I cannot understand how in this age of computers, it is not something
that can be resolved without the need for a dongle.
