Andy, have you tried adjusting the volume via the alt-control arrow keys? Press alt-control plus left arrow until you hear volume, then use the up-down arrow keys to adust the volume. If this works, then you will have to do a save in the Window-Eyes control panel, under set file to retain the setting. This seems the most likely cause of the problem to me.
