I would try and get a shortcut key for JAWS 9.0 created so you can try
the form with that instead.  Jaws 10.0 is beta and we cannot support
that at the moment.  JAWS 9.0 is more of a known quanitity. However it
may be that it is a problem with the form.

Here are some notes for the shortcut key.

Creating a shortcut key to start JAWS

Some situations may require that you unload Jaws from memory and then
need 
to restart Jaws.  The easiest way to do this is using a shortcut key to 
attach to the start icon for the software.  Below are the instructions
for 
doing this.

There are two ways for you to create a shortcut for Jaws, the first is
on 
the desktop and the second on the start menu.  If you find that the 
keystroke you have assigned does not work on the desktop then you should

delete the desktop icon (for ease at least) and then us the second set
of 
steps to create it within the start menu group icon for Jaws.  One of
these 
methods usually works.

1    Press windows+M to move to "desktop".
2    Press letter J until you reach the Jaws icon on the desktop that
starts 
your version of Jaws.
3    Press alt+enter to open the icon properties for Jaws.
4    Then tab twice to the "shortcut key" edit field and type letter J
into 
it.
5    Tab to "OK" button and press spacebar on it.
6    Before closing Jaws press windows+m to go back to desktop and ten
press 
control+alt+J.  If after a short perdio hear "Jaws" and the Jaws window
has 
popped up this means you shortcut will work for you when you try it for 
real.

Alternative method for setting hotkey for Jaws where desktop shortcut
does 
not appear to work
1    First delete the icon for your shortcut you set up previously or
press 
alt+enter and make sure in the "shortcut key" edit field that Jaws is no

longer assigned to control+alt+J.
2    Open start menu and press letter P for "programs", then press J for

Jaws.
3    Press shift+f10 for context menu and up arrow until you get to 
"properties" then press enter.
4    Then tab twice to the "shortcut key" edit field and type letter J
into 
it.
5    Tab to "OK" button and press spacebar on it.

Notes: You can remove Jaws from memory by pressing insert+F4 and press
enter 
on the "OK" button.  Then you will be able to use your newley setup
shortcut 
key to riase Jaws again.
