In answer to the question "why is it Americans always come first?"

America is many multiple times the size of the UK market, it makes business 
sense for Amazon as an American company to deal with their home market first 
before venturing into other territories.  The UK market though it is English 
speaking is not entirely the same as the USA and is also tiny by comparison and 
is also more expensive market by virtue to enter initially.  Crossing over into 
Europe means a raft of different legislative and legal demands that must be met 
by the American company when trading overseas because a company is itself a 
legal entity.  Most of the large book publishing and software firms are 
American based or have substantial business markets in the United States and 
therefore it is easier to negotiate rights in this and test their product in 
that market, make some money and then throw out the net a little wider.
