Ideally you want to contact the developer of the innaccessible app rather than Apple themselves. They can only pass info onto the developer which is possibly why they asked you to fill in the form. I you can find the developer on the AppStore there is usually a link to them somewhere and you can find a way to make your views known rather more directly. Regards.
