If its any help at all what I do is:
Log onto the site and hit the log on link
Hit the next button to get to step two
Put the last four digits of the card in the first box then tab to the box to enter the first four digits from the card reader. At that point i shove the card in, enter my pin number and put the cardreader on the desk. Then I hit enter on the card reader and interactively type in the cardreader code. This works for me, initially I did have problems becasue I was taking too long to enter the code.

Probably less than useless but thaught I'd share my method with you.

I just thought I'd let you know that I've just had four more tries at getting this Pinsentry system to work. The first two were again abortive in the same way as previously. However, I did get in on the third try and so came out and tried again, with success this time as well.

So, perhaps it's going to behave itself reasonably, if not perfectly, from now on.
