Unfortunately it's not an extreme example at all and it seems most common in large Advertising and PR agencies. I've heard so many colleageues bring up this very issue and I know that many have tried to institute a process and educate the internal clients but I've yet to hear a success story and usually to the detriment of the client and project and usually to the teams immense frustration.
