I think Ari, hit it pretty well. Just to add how we do it...
First, I've been doing this type of work for over a decade now, so I  
have quite a few projects to base time off of. That being said, we  
typically estimate by the following method:

1. Client sends out an RFP, which outlines what they want and need.
2. We review the RFP and begin thinking about previous work we've had  
that matches up to that from the perspective off:
   a) process
   b) type of work (web, application)
   c) methods involved (research, IA, usability, design, HTML/CSS  
production, project management)
   d) volume and duration (how big, how long)
3. We dig up an old response to RFP from a similar project to use as  
a template.
4. We go through our project timing and billing application (we use  
iBiz by IGC Software http://www.iggsoftware.com/ibiz/ - great little  
app that works with QuickBooks) and look at our estimates against  
actually billings.
5. We formulate an estimate based off of a combination of the  
previous response to RFP and actual billings performed for similar  
projects.

Our process typically includes:
1. Research - user research, task analysis, audience definition,  
personas
2. Design - task grids, taskflow diagrams, sitemap, wireframes,  
visual design
3. Production - HTML/CSS templates

As Ari suggested, we typically add in a bit of padding (10% or so)  
into the timeline and a few of the line items to make room for things  
like: client isn't around when we need sign off and causes a couple  
day delay, client wants extra revisions, client "forgets" something  
and adds it in later in the project, etc.

I've seen two methods for project management costs: 1) actual  
estimated hours 2) 10% of the total estimated cost of the project.

Once again, rates are dependent upon discipline, experience, and who  
you are. We've seen rates from $40-180, typically, but some larger  
"Big 5's" like Deloitte and KPMG can get $300/hr for work.
