I've heard of chumpsofts phpq, which is inexpensive downloadable and easy to 
use.

For enterprise level I have heard of key survey and qualtrics, but is more more 
powerful scalable and expensive.

I have not used any of these yet though. I Have heard great things though about 
chumpsofts one.

Hello,
Our company is looking for a website to build a survey that will place users
into one of three (or more) choices/categories...very similar to a cosmo
quiz. It needs to be customizable, ad free (if possible) and embedded on our
site. The simpler to use, the better. A paid service is fine.
I'm looking into surveygizmo.com and formspring.com. Any other great sites
out there?
