Digital Media, we used Survey Monkey.

I am a huge fan of Demographix (http://www.demographix.com). It is a highly 
ustomizable tool and the customer support is unparalleled. I have been using

 Our company is looking for a website to build a survey that will place users
 into one of three (or more) choices/categories...very similar to a cosmo
 quiz. It needs to be customizable, ad free (if possible) and embedded on our
 site. The simpler to use, the better. A paid service is fine.
 I'm looking into surveygizmo.com and formspring.com. Any other great sites
 out there?
