I checked around the office and nobody has any recollection of an inquiry like 
yours in the last 2 days, so I'm sorry you got the impression it wasn't 
available. SurveyGizmo does offer the type of quiz you are looking for at our 
PRO level (not available for free). It's called a Tally Quiz, and here is an 
few examples for you:
