Rob, thanks a ton for that info, I was hacking on this all weekend and was pretty pessimistic that this could be done, we have an app in the picture also so the approach you outlined should work for us.
Nope, never got that to work. However, since my driver comes with an application, I set the icon info programmatically by implementing IMMNotificationClient, and on OnDeviceStateChanged handler I set the PKEY_DeviceClass_IconPath property of the device to point to the driver .sys file (since the icon is in the driver as an icon resource).
