We haven't found anyone who has one that we can look at for ideas. We are
hoping to make a form with drop down boxes for recording information. Many
times we don't need much of a narrative and search for words to write something
to satisfy the requirement, as the students is working through the same lesson
or goal for sometime.

Currently, our Center operates in the same manner as your agency that
is that each instructor develops and uses his/her own evaluation form. From
there, their evaluation report is written in a narrative form. I like the idea
of a checklist for documenting the client's progression of rehabilitation. As
of yet, I have not been able to locate any type of checklist for assessing
one's rehabilitation program, have you?

Michael; I work for the South Dakota Rehabilitation Center for the
Blind in Sioux Falls. I am a communications instructor. We are working on
doing an evaluation and ongoing recording form for training. Right now, each
teacher has their own and work off that and then we enter things in narrative
form into a comments section in our reporting document.
The idea is to move to some kind of a check list that would show a
progression of training, without getting too complicated, and then having a
small comments area for further explanation.
What does your center use for evaluating and recording progress notes?
