The nearest I get to that, is when I unplug my headphones - a non-Apple variety - but good quality retractable, stereo walkman-style - the sound will pause. (It has stopped 'playing'. I just two-finger swipe, or double-tap the 'play' button, and whatever I was doing resumes at the same volume level as before.

I think you have this issue sorted but i always loose some sound with voiceover when i'm using non apple earphones. I cried the first time it happened as couldn't get a peep out of my phone i had to switch it off.

You have sound effects on?
Is the volume of the Sound Effects in VoiceOver affected? Or is it only the
Voice?
Have you tried changing voices?

Have you tried it with a different pair of headphones?
Is your usual pair a normal plug-in pair or a Bluetooth pair?
Have you got more iPhone sound volume on one side or headphone than on the
other?

Well the volume seems to have something to do with it because when I change
the volume the sound comes out through the speaker and then it fixes itself
for a while until I put in the headphones again.

My volume is set to 100 but like you say sometimes when I use the
headphones
it can go down to around 30 percent. The odd thing I have noticed is that
when I turn up the volume VoiceOver comes out through the speaker again.

It is very strange!

I have noticed a few times that the audio volume drops to zero on my
iPhone
5S. Particularly after using the headphones. I don't know why this is. It
means that I cannot hear a phone call until I go into settings and
"Sounds"
and bring up the volume. I like to set the volume in "Sounds" at 100% and
then use the volume keys on the side of the phone to adjust it.

I cannot find any reports of this phenomenon nor of your problem.

Let's hope someone comes up with a fix or an explanation.
