Hello,
You can have a child with both, though it is extremely unlikely that they would
be full T18 and T21. It is much more likely that it would be a partial or
translocation of parts of each chromosome.?Just like with Tripoidy, in which
every chromosome?has an extra third copy, it is possible to have multiple
trisomies. ?
However, it sounds to me (though I am not a doctor, I am a midwife who is
currenly working on her Physician Assistant masters in US and has studied fetal
abnormalities) that this child's problem is much more likely to be Fetal
Alcohol Syndrome, even with the screening risk increased for T18 and T21. The
screening blood test looks for placental hormones in mom's blood. Based on the
levels in mom's blood, when compared to the "mean" or middle average of women
of the same age, you get a risk ratio (1 in 23 for example). Those hormones are
also skewed in cases of placental previa, or other abnormalities that CAN come
from things like severe alcohol abuse (80oz a day is extremely severe and
almost impossible to believe) and effects of drugs and toxins. The baby's
ultrasound findings can all also be fetal alcohol effects as well (short femurs
for example show poor growth, and are not just a sign of T21, but a sign of
fetal growth issues as well).
It sounds to me though, that regardless of what this child may or may not have
you are willing to welcome it. If it does have a FAS, the effects can be severe
to mild and each child is extremely varying but there are some classical
findings. The amount of alcohol plays a big part in the severity.
If this child has T21, as I doubt it would have T18 based on the ultrasound
findings, it would still most likely have a fetal alcohol syndrome as well, on
top of the T21 issues. This is important to recogize and plan for if that is
the case.
Here is a resource about FAS to review and discuss with your doctors to help
you to decide what you would like to do.

?hope this helps, and feel free to email me off list if you need to

I know you don't know me from Adam, but my thoughts are, do you really want to
have a baby with all those problems?
And no, you cannot have a baby with both Downs and T18.? My risks with Rosemary
were high for both but the genetic counselor said that they can't have both.
My best wishes go out to you as you struggle to make this decision.
Hugs
Karen

Hi
I was hoping to get some help with a few questions!!
We have been chosen for an unborn baby that is due at any time...Birth mom
is in the hospital on bed rest until delivery due to placenta previa
....Here are the details we would love input on....
----AFP Quad Screen: Risk for Down syndrome: 1:37...with maternal age 1:29
Risk for Trisomy 18: 1:23
(they are saying that the baby will has Down syndrome (which we are thrilled
about!! LOL) BUT how can they say that if there is a risk for Trisomy 18 as
well????---can you have both or will it be one or the other?? A amnio has
not been done yet....)
----There is no noted heart defect BUT shows shortened femurs, midly dilated
bowel and a mass superior to the right kidney
----Smoked 1-2 packs of cig. a day for the first 20 weeks and then reduced
that when she found out BUT not sure by how much...and she is on a nic.
patch while in the hosptial per the doctor....
----Drank 80 ounces of alcohol PER DAY for the first 20 weeks and then
reduced it to 24 ounces of alcohol PER DAY since then...since June 28th when
she was admitted to the hospital the alcohol has stopped....
----Urine Drug screen was postive for: acetaminophen, nicotine, opiates,
propoxyphene and cannabinoids
These are the things that have stuck out in the medicals that we just
received and I would LOVE thoughts and advice and experience etc....thanks
in advance.
