You would call Vital Statistics in your County and tell them you want to  
find out how to amend a death certificate because of incorrect cause of death. 
 
You then call the doctor who signed it and explain to them what you want to  
do, and why requesting that the death certificate specify her heart condition  
and actual cause of death and list Trisomy 18 as a contributing factor.
