Trisha,
I think you will find that its just a pretty standard counselling form that
most geneticists use for an initial consultation. It saves time if you can
go into these meetings with all the information available that they may
possibly need for a whole range of problems. They send them to everyone when
they set up the appointment.
I got a similar form when we went for Alex's follow up consultation (and
there is no way that anyone could be a carrier with Alex given the
mosaicism). I did complete it as best I could and its turned out handy for
family tree research stuff :-))
