- which resulted in my 2 year old Matthew. If you would like me to
answer for my 2nd (miscarriage) and 3rd (Grace - full T13), just let me

1. In what country to you live?
2. What year did you first fall pregnant? (this is the very first time
you EVER fell pregnant, not necessarily a tri pregnancy)=20
3. What was your age when you FELL pregnant?
4. Where you aware PRIOR to falling pregnant that there were tests done
or that could be done during the pregnancy to detect the health status
of your baby?
5. If the answer to Q2 was yes - How did you become aware?
eg - school biology lessons, friends who had become pregnant, your
doctor explained this, pre-existing familial genetic problem etc Don't
know is an acceptable response.
6. What tests were conducted during this FIRST pregancy?
a) CVS
b) Blood screening
c) Amniocentesis
d) Ultrasound
e) Cordocentesis
7. Where any problems with the baby identified? If yes please detail
briefly what these concerns were (eg identified high risk for genetic
problems, identified choroid plexus cysts etc)
8. What was the outcome of the pregnancy?
a) Live birth
b) Stillbirth
c) Spontaneous miscarriage
d) Termination
9. Was this birth related to a subsequent diagnosis of a chromosomal
anomaly?
