Have you had the 24 hour test to see if she has reflux?  Nicholas used to wake 
up screaming and gagging, so we took him to a gastro and they put him in the 
hospital and did the probe for 24 hours.  He was on reflux meds before that and 
the screaming stopped for a while, but then he started it again (not as bad, 
though).  We then went ahead and did the fundo and the gagging and screaming 
has stopped - at least for that reason!  He still yells when he wants attention 
or out of his bed. :) which is usually at 3:00 a.m.!
