Don't know if this would work or not but could you have a Pediatrician as
the 'head dr' who would then somewhat control/direct the Neonatologist?

Make sure that you and your signifigant other are on the same page. It's a
huge benefit in your decision making, esp if things are happening fast and
furious after birth, if you have discussed with EACH OTHER what you want for
your child.
Also, if the drs have their own agenda and sense a chip in one parent or the
others resolve it might sway them to not follow wishes.

Birth plan ideas:
If you are planning to breast feed put that in there.
Discuss comfort measures with your dr and see what they include and what you
might want in the event that they are more appropriate than full
intervention (you decide at the time).
Also, what are your wishes are should Tyler not survive birth: religious
rites, family called to spend time together with Tyler, etc
Have your own plan of momentos you may wish to create in the hospital--pics
with any/all family members, footprints in plaster of paris, etc. so that if
need be you can do it quickly and in the moment.
