I guess you could say they have good ears, and since it ( the heart) is
their speciality they know what they are looking for,  the reason not
hearing a murmur can mean it is not closed is because the sound they hear is
really the sound caused by the force of the blood through the defect,  a
large hole allows lots of blood through the defect ( think garden hose
running full open) since there is no opposition to the blood getting through
it is relatively quite, a small defect does not allow allot of blood to get
through so the force of the blood is greater ( think how the water comes out
of the hose when you put your finger partially over the opening) this is why
is sounds louder.  A dr can't tell for sure that a heart has healed or not
by just listening, but depending on the type of defect they know what the
odds of it healing on its own are and this coupled with what they hear is
how they decide/or they can do an echo.  using Rebecca as an example, when
she was young they followed her every year and she had an echo most times,
now she is seen every 3-4 yrs, she still has the hole (VSD) but it is small
and membranous and will not heal on its own,  when you listen to her heart
the murmur is very loud, I think the reason they only see her every 3-4 yrs
is that they don't plan on fixing her heart as it does not give her any
trouble and is not life threatening at this time, and that since she has
been stable for so long they don't feel that frequent follow up is necessary
