when the hole is small, there is  an amount of force needed for the blood to
get through  the hole, when the whole is large there is less pressure, kind
of the difference between  putting your finger over a garden hose or not,
this is why you can hear a small murmur but not a large one,  pulmonary
hypertension is not one of my strong subjects. there are many here who have
experience with it though
