Here in Australia we tend to use the Goanna chair which I don't think is

It's actually called a "squat position" - knees should be higher than the 
hips - in fact I believe that we should all use the toilet in this position 
as it allows you to "push" naturally. People may laugh but I have kept 
Alex's toilet "step" in our bathroom and thats used by all in this house. 
There are however arguements for and against "the squat", I just happen to 
be in favour :-)) While looking for pictures of a squat toilet I even saw 
that Wikipedia has the squat arguement listed.

There is a commercially made toilet that incorporates this - not the Asian 
variety - its a regular toilet with a step around the bottom.
I found a picture of a "converter" at 
http://www.pelvicfloorexercise.com.au/products/inlieu.htm but the one that I 
have seen is actually the toilet specifically made with the step around it. 
As I said though we just use the plastic step. In fact its pinched as a 
quick step for the tops of cupboards and to go on top of a chair for me to 
change light bulbs all the time as well.
