Welcome.  Our daughter Mari is a gagging-puking child too!  I can't say that
I have any advice, but do share the same frustration.  No pattern to hers
either.  She sometimes gags & throws ups BEFORE we even get her formula in
her!  All Mari's tests show no actual obstructions.  I also think hers is
related to slow digestion (constipation) issues.  Mari's seems to be related
to mucus too!  The snottier she is...the more she pukes.

We haven't checked Mari's tonsils, but her ENT didn't think there was
anything in his department causing the vomiting.  Mari also has mild sensory
issues.  I don't notice her vomiting more in certain positions.  Mari gags &
gives back every single feeding at some point.  We don't think it's volume
related.  My theory is hers is part behavioural & part neurological.  Her
system needs a "do over" every time she eats.

We've used Miralax as well.  Mari has also been on Reglan for reflux since
she was four months old (same time the gagging/vomiting began).  I'd love to
hear what else you've tried or are going to try!  Sorry, we can't help!!
