[tri-med] Nicholas Update 4/26

  a.. Nicholas was taken off of mechanical ventilation around 3 PM today and 
started to breath on his own. He is getting 40% oxygen at 15 liters flow with 
moisture through a nasal cannula and is doing well at present. 

  b.. Breathing treatments and/or medicine will be needed in the next 
hours/days to help with the effects having the breathing tube in for 4 days. 

  c.. Tonight they will start G-Tube feeds slowly (10 ml/hr) with TPN. 

  d.. Nicholas is off all pain medication at present. 

  e.. Nicholas is opening his eyes more and squeezes your finger when placed in 
his hand. 



As we were writing this update at 7:15 pm Nicholas started to have trouble 
breathing after being turned in bed.  He got excited and anxious which led to 
stridor. (difficult breathing due to inflammation of the muscles in the throat 
area)  It was a scary event for us as his respirations went up and oxygen went 
down to critical levels.   They stabilized him by giving him Decadron, Adavan, 
Ephinephrine and starting Heliox at a 60/40 mix.  Nicks blood gasses came back 
fine.

He is stable right now (9 PM) but we are on edge. 

We were told that this isn't uncommon and in older children peak swelling in 
the throat area can happen 4 or more hours after the breathing tube is removed.

Nicks attending physician in the ICU is Dr. Mitch Ross who received his medical 
degree from the University Of Utah School Of Medicine.  Dr. Ross knows Dr. 
Carey and I overheard him tell another doctor that Dr. Carey is like a walking 
encyclopedia. 

Dr. Carey, please tell us that Dr. Ross got good grades in school?!
