Janet,
We were able to go through the process smoothly.  Like Fawna mentioned, 
we had a school psychologist's report, plus had to include all sorts of 
info about our income, etc.  The school letter has to be updated once a 
year before you reapply (you have to send in paperwork each year).

The biggest tip that our school PT gave us was that when you do send 
the information in, you take it to the post office and ask for a return 
receipt.  That way you know the state gov. office got it, and you have 
the proof in case they say, "But, but, but....."  We've done it twice 
now and had a response quite quickly.  The monthly checks help out a 
good bit for us!
