Our insurance company initially told us they wouldn't approve our T-18 
daughter's formula also.  We found a clause that stated if the formula was her 
sole source of nutrition and prescribed by a doctor due to a birth defect, it 
would be approved.  Each year the insurance company denies it & our doctor has 
to send a letter explaining why she needs it.
