Aren't orthopedists just the cockiest people? (sorry if I am offending
everyone, there are exceptions I am sure). I ran into similar answers
in my 25 years of dealing with them.
It sounds as if your approach is the best for Kam. Just one thing I
might add that really helped me since I have had a dislocating kneecap
since the age of 6. If Kam can do any type of straight leg knee lifts
you should have her do them or at least help her do them because the
stronger the thigh muscle gets, the less the knee dislocates. It
really does wonders for me. She can even do them with an ace bandage
on so that the kneecap stays in place more while she is doing them.
